Part B Premium Reduction (Part B Giveback/Cashback Benefit) is not available with all plans. Availability varies by carrier and location. Actual Part B premium reduction could be lower.
Healthy Options Allowance, Extra Debit, Chronic Condition Care Assistance, Flex Card, and Music Therapy may be part of a special supplemental program for chronically ill members on Special Needs Plans with one or more qualifying conditions, such as: diabetes mellitus, cardiovascular disorders, chronic and disabling mental health conditions, chronic lung disorders, chronic heart failure. Members on other plans must have two or more qualifying conditions. This is not a complete list of qualifying conditions. HUD requires people who use plan benefits, including but not limited to Healthy Options Allowance, Extra Debit, Flex Cards, and Chronic Condition Care Assistance, to pay rent and/or utilities to include it in the calculation of income.
